The husband of a Sydney woman accused of "purposefully" reversing over a cyclist before fleeing the scene of the fatal incident has apologised to the 66-year-old victim's family.

Vonny Ho, 47, was granted bail in Parramatta Bail Court on Saturday after being charged with the fatal hit-and-run of the cyclist in Sydney's west on Tuesday afternoon.

Police accused the 47-year-old of "purposefully reversing back towards" and over the cyclist, before failing to stop after the incident on Harry Avenue in Lidcombe.

The 66-year-old male cyclist died at the scene.

In court a lawyer for Ms Ho claimed his client only learnt of the incident after seeing it on the news on Thursday night.

She has been granted conditional bail.

Ms Ho's husband Effendy Chang was in court and offered a $10,000 surety with the condition of bail.

Speaking outside court, Mr Chang apologised to the 66-year-old's family on behalf of him and his wife.

"She's remorseful," he said.

"We are really sorry."

Ms Ho will reappear before Burwood Local Court on Thursday.

M4 fatal crash charges

In a separate incident, a motorist allegedly responsible for a fatal pile-up on the M4 on Thursday was charged with a number of serious driving offences.

The 53-year-old man, due to appear at the Parramatta Bail Court on Saturday, allegedly drove a Toyota Echo on the M4 when he changed lanes and stopped at the city-bound Church Street off-ramp about 5.50am on Thursday.

He allegedly left the scene before speaking with police.

Francis Shanley, a 36-year-old Irish tradesman, died in the crash.

The 53-year-old man handed himself in to St Marys Police Station about 6.15pm on Friday.

He was arrested and charged with dangerous driving occasioning death, negligent driving occasioning death, failing to stop and assist after the vehicle impact, causing death.
