Child protection services were deficient in the way they dealt with a Queensland toddler neglected and beaten to death by his guardians, a court has heard.

An inquest will aim to understand 'what was going on the minds and hearts' of child safety officers tasked with protecting Mason Jet Lee.

The actions of authorities before the little boy died will be the subject of a coronial inquest in Brisbane in coming months.

At a pre-inquest conference on Thursday, Brisbane Coroners Court heard department officers had been working with Lee to meet her son's care needs but their actions fell short.

'An ethical standards investigation into the actions and decisions of individual child safety officers following Mason's death was critical of a number of the officers involved,' Megan Jarvis, counsel assisting the coroner, said.

She said the inquest would explore officers' 'incredibly challenging' decision-making around the case.

Lee and O'Sullivan pleaded guilty to Mason's manslaughter after O'Sullivan fatally struck him in the abdomen and both failed to get him medical treatment as he suffered an agonising death.

Harrowing details of Mason's 'survivable' injuries - inflicted by O'Sullivan about five days before his death - were heard on Thursday.

He would have been in pain, vomiting, dehydrated, feverish, lethargic, pale and displaying 'impaired levels of consciousness (and) altered breathing patters', Ms Jarvis said.

'It would have been obvious to any normal person he was very sick and in urgent need of medical attention.'

By the time O'Sullivan called paramedics, Mason's body was displaying signs of rigor mortis.

The inquest is expected to be held in November and December.
