The wall power adapter for the Tandem t:slim X2 Insulin Pump is faulty and could break apart, exposing the metal prongs.

The adapter was supplied by Australasian Medical and Scientific Ltd.

"If the exposed metal prongs are touched when the adapters are plugged into an electrical wall outlet, this could cause an electric shock or serious injury," the ACCC said in a statement.

"Consumers are advised to discard or destroy the three power adapters that they received when they initially purchased their Tandem t:slim X2 insulin pump.

"Australasian Medical & Scientific Ltd advises that along with the consumer letter, affected consumers will find a replacement adapter that has been manufactured under the new, corrected process."

The power adapter was available for sale nationwide from the start of October last year to the end of October this year.

The Therapeutic Goods Administration are the regulator responsible for the recall decision.

Last month the ACCC revealed that about half of the 6.6 million consumer products that are under recall remain in people's homes.
