Tributes have poured in for Italian race car champion-turned-Paralympic gold medallist Alex Zanardi after he crashed his handbike into a truck during a race in Italy.

Zanardi is in a medically induced coma and remains in serious condition after the crash, which happened near the Tuscan town of Pienza during the Obiettivo tricolore relay race on Friday.

The 53-year-old — who lost both of his legs in a motor racing crash nearly 20 years ago — underwent a three-hour operation for severe head injuries.

"The condition of Alex Zanardi is serious but stable," Zanardi's surgeon at the Santa Maria alle Scotte hospital in Siena, Dr Giuseppe Olivieri said.

Ex-Formula One driver Alex Zanardi , who had both legs amputated in a motor racing accident almost 20 years ago before going on to become a Paralympic champion, was in an "extremely serious condition " in hospital on Friday after crashing into a truck while competing in a handbike race in Italy.

After two spells in Formula One either side of a successful CART stint, Zanardi returned to the North American open wheel series, where he severed both his legs in a shocking accident in 2001.

During his recovery, Zanardi designed his own prosthetics and learned to walk again.

He then made a sensational return to motorsport, winning four World Touring Car races using a specially modified car.

That inspired others to follow, including British driver Billy Monger, who won his first race just two years after losing both his legs in an accident.

Monger lead tributes that included Italian Prime Minister Giuseppe Conte, who tweeted that "All of Italy is fighting with you."

After his motor racing career, Zanardi turned his attention to hand cycling and developed into one of the most accomplished athletes in the world.

He won four gold medals and two silvers at the 2012 and 2016 Paralympics, competed in the New York City Marathon and set an Ironman record in his class.

Criminal investigation opened

Prosecutors in Siena opened a criminal investigation into the accident and the race's organisation — specifically whether enough safety precautions were taken.

About 10 disabled riders were escorted by one police vehicle, local reports said.

Witnesses said the accident occurred on a downhill section just before a curve in the road. Zanardi apparently lost control and veered into the oncoming lane.

The truck apparently tried to swerve out of the way but couldn't avoid the crash.

The truck driver, who was placed under investigation, tested negative for alcohol and drugs.

"The truck didn't make a mistake. Alex made the mistake," said national team coach Mario Valentini, who was following the race.

The race was part of a two-week relay event throughout Italy to promote the country's revival after the coronavirus pandemic. It was created by an association led by Zanardi with an aim of bringing more Italians to the Paralympics.
