Disturbing questions are being raised a fatal boating incident in Canada involving "Shark Tank’s" Kevin O’Leary.

A 48-year-old mother of three and a 64-year-old Florida accountant were killed when the boat they were on collided with O’Leary’s.

The accident happened late one night on a lake in Ontario, famed for its multimillion-dollar waterfront vacation homes.

O’Leary said that his wife was driving the boat and that the other, much larger boat, did not have navigation lights on. But a witness claims O’Leary’s boat may have been travelling too fast.

A representative for O'Leary told the celebrity website that the former "Dragon's Den" star's wife was driving the boat at the time. Provincial police say two boats crashed on Saturday on Lake Joseph near Seguin, Ont., killing a Florida man identified as 64-year-old Gary Poltash. The force says a 48-year-old woman, identified as Susanne Brito of Uxbridge, Ont.
