86-year-old woman dead after four-vehicle crash on Highway 401
An 86-year-old has died following a four-vehicle collision in the Hwy. 401 westbound express lanes, approaching Leslie St., on Saturday. Ontario Provincial Police were called to the scene around 4:25 p.m., said Const. Lauren Ball. One person was transported to hospital with life-threatening injuries, she said. That person later died in hospital. In a tweet, police said the deceased was an 86-year-old woman. Ten other people, including three children, were also taken to hospital with non-life threatening injuries, said police. The westbound express lanes are closed at Warden Ave., and the Hwy. 404 southbound ramp to Hwy. 401 westbound express lanes are also closed.
The driver of a transport truck is now facing charges in connection with a a crash on the Queen Elizabeth Way last month that killed a 34-year-old Niagara Falls woman.
It happened on Aug. 22, a short distance east of the exit for Trafalgar Road.
Police say one of the vehicles involved — a transport truck — caught fire as a result of the crash and the eastbound QEW was shut down for several hours for the investigation.
In a news release issued Monday, Ontario Provincial Police said the driver of one of the cars, 34-year-old Elena Kulikova of Niagara Falls, died at the scene.

Police said officers were called to a house on Chisholm Avenue in East York at 4:16 a.m. for reports of a crash. Images from the scene showed a black Mercedes sedan that crashed into the front of the home, causing significant damage. READ MORE: Toronto police arrest 2 men after car crashes into Etobicoke home "I was asleep upstairs in bed and it was about 4 a.m.," Carolyn Cockburn, who lives in the house, told Global News. "I could hear a car kind of skidding and I could tell there was something going fast around a corner and then there was a big 'boom.' The house shook and my smoke alarm went off.
Several other people were also treated for non-life-threatening injuries, police said.
A 34-year-old Mississauga man has been charged with dangerous operation causing death and causing death by criminal negligence, police said Monday.
He was held for a bail hearing on Sept. 13 in Milton.
