Next, Miriam explores the world of extreme weight loss, meeting those who have had operations on the NHS to halve the size of their stomachs in order to lose weight. While often successful, there are other complications. Not only do they have to undergo major surgery, but they must also deal with the large amounts of excess skin. Some also struggle to then see themselves as slim and develop body dysmorphia.

Lastly, she meets Aeronn who has lost four stone in just 25 weeks and is competing in the Natural Body Building championships in Newport, Wales. Miriam also meets 16-year-old Bart who, despite having an impressively toned physique, is desperate for bigger and better.
