Santa Fe County Sheriff’s officials said Halyna Hutchins, director of photography for the movie “Rust,” and director Joel Souza were shot. Hutchins, 42, was airlifted to University of New Mexico Hospital, where she was pronounced dead by medical personnel, authorities said.Souza, 48, was taken by ambulance to Christus St. Vincent Regional Medical Center, where he’s undergoing treatment for his injuries.

An assistant director unwittingly handed Alec Baldwin a loaded weapon and told him it was safe to use in the moments before the actor fatally shot cinematographer Halyna Hutchins, according to court documents.

The unnamed director did not know the prop gun was loaded with live rounds and indicated it was unloaded by shouting “cold gun!,” according to a search warrant filed in a Santa Fe court.

The details are the latest in a police investigation to piece together the events leading to Ms Hutchins, 42, being shot in the chest by live ammunition on a film set in New Mexico on Thursday.

Cinematographer Hutchins, named one of the industry’s rising stars, died after Alec Baldwin fired a prop gun while filming western film Rust. Director Joel Souza, who was standing behind her, was wounded.

The notes state that Baldwin’s costume was taken as evidence, as was the weapon that was fired.

Mr Baldwin, 63, was questioned by law enforcement, but no one has been charged over the incident.

Details have emerged that crew complained about gun safety on set, with experienced workers walking out hours before shooting in a dispute over working conditions.

The producers of Rust replaced union crew members engaged in a work stoppage with non-union substitutes hours before the incident, the Los Angeles Times reported.

A half-dozen of camera crew workers walked off to protest working conditions. The workers, who said they struggled to collect their pay, worked long hours and faced lengthy commutes for the low-budget film.

They said gun safety protocols standard in the industry, including gun inspections, were not strictly followed. A worker told the publication one of the camera operators warned a production manager last weekend they were concerned about gun safety on set.

BBC News obtained a document showing which crew members were listed as scheduled to be on set that day.

It names a head armourer – the crew member responsible for checking firearms – who is in her twenties and had recently served in that role for only the first time.

The prop gun that Baldwin fired contained a “live single round”, according to an email sent by the International Alliance of Theatrical Stage Employees to its membership, reports Variety.

Ms Hutchins, 42, was from Ukraine and grew up on a Soviet military base in the Arctic Circle. She studied journalism in the capital of Kyiv, and film in Los Angeles. She was named a “rising star” by the American Cinematographer magazine in 2019.

The role was her most prominent following her position as the director of photography for the 2020 action film Archenemy, directed by Adam Egypt Mortimer.

Mr Baldwin was seen crying outside the Santa Fe Sheriff’s office, distraughtly clutching his phone on Thursday evening.

In a statement, he wrote “there are no words to convey my shock and sadness” regarding the tragic accident.

“My heart is broken for her husband, their son, and all who knew and loved Halyna,” he wrote on Twitter.
